Patients often come to the assessment and rehabilitation period from hospital following treatment. You can also start the period after staying at home if the necessary assessment and rehabilitation cannot be carried out at home.
We will support your functional capacity during the period. We will also assess what kind of support and care you will need going forward.
The assessment and rehabilitation period is a fixed-term, around-the-clock short-term care period. The aim is that by the end of this period, you will be better able to cope with everyday life and will be able to return home with the support of the necessary services.
How to get to the assessment and rehabilitation period?
Patients can be admitted to the period following hospital treatment or directly from home, when assessment and rehabilitation cannot be carried out or continued at home. You may be referred for an assessment and rehabilitation period if this is deemed necessary during a service needs assessment.
If you are already using the City’s senior services, such as home care, please discuss your assessment and rehabilitation needs with your home care staff or your designated worker. How much does a rehabilitation period cost?
We charge a short-term care fee for fixed-term assessment and rehabilitation periods.

The duration of the period is tailored to you and depends on your needs.
What should I bring with me for the assessment and rehabilitation period?
Personal documents
- Kela card
- ID card or other form of identification
- Up-to-date medication list
- Taxi card, if you have one
Medicines
- All your regular medicines and any medicines you may need to take, in their original packaging
- Diabetes supplies or other medical supplies, if you use them
Clothing and footwear
- Comfortable indoor and outdoor clothing that can be washed at 40 degrees or higher
- Sturdy indoor and outdoor shoes
- Nightwear
- Enough changes of clothes
Don’t forget to label all your clothes!
Toiletries
- Personal care and hygiene products and protective products
- Toothbrush and toothpaste
- Shaving equipment
- Comb
- Any skincare products
Assistive equipment
- Glasses, hearing aids and their batteries
- Mobility aids you use on a daily basis (e.g. a walker or walking stick)
Other necessary items
- Home keys
- Phone and charger
- A small amount of cash for travel to and from home visits (and possibly for a pedicure, a barber or a visit to the café)
- You can also bring your own magazines or books, for example.
It is advisable to leave valuables, such as jewellery and expensive electronic devices, as well as your security wristband, at home.
