The memory coordinator will assess your functional ability and memory and update the plan for maintaining your functional ability together with you. We can refer you to services that will support your ability to function.
The services of the memory coordinator are intended for people with a memory disorder who live with their loved one. Home care employees attend to memory-related matters of home care customers.
We meet our customers at the Geriatric Outpatient Clinic, arrange remote appointments, can call you by phone or visit you at your home.
