Service plan

Disability services social work promotes the ability of disabled persons to cope on their own and seeks to diminish restrictions and obstacles caused by the disability.

Disability services social workers are experts in services related to the Disability Services Act, and they cooperate with home care, hospitals and various associations. The social workers can be contacted if the disability or illness causes problems with living, mobility and coping at home.

Disability services social workers are experts in the organisation of services for disabled persons. The social workers are usually contacted when a child is discovered to have a disability or when an adult disabled person needs services related to work, living or other.

A customer and the disability services staff work together to determine the customer’s service need. An evaluation of the need is used to prepare a service plan.
