Service plan

A service plan, as defined by the Disability Services Act, includes the descriptions of the current situation, functional abilities and assistance need. In addition, the current services, their implementation method and the need for potential new services are written down and, furthermore, the persons in charge of executing the support measures and the next update to the plan are agreed upon. A social worker from the social work for the disabled will compile the service plan.

The service is applied for via the social worker of the disability services at the social work services for the disabled of the customer's own residential area. Swedish-speaking customers should apply for the services via the Eastern Office of the social work services for the disabled, where they can receive service in Swedish.

The services are free of charge for the customers.


Service needs are assessed, together with the customer and his or her close network. The service plan is not a decision concerning the support measures presented in the plan. Instead, they have to be applied for separately and a positive decision must be received before the service is executed or the device purchased.

The service is only for citizens of Helsinki.
